What may be related to the prostate
Prostate adenoma is a common term for benign prostatic hyperplasia. An enlarged prostate can make it harder for urine to leave the bladder. However, symptom severity is not determined by gland size alone: its structure and the function of the bladder itself also matter.
Reasons to see a urologist include an intermittent urinary stream, straining, a feeling of incomplete emptying, frequent or sudden urges to urinate, and dribbling after using the toilet. Getting up at night can also be related to fluid intake, sleep disorders, diabetes and other conditions. Treatment therefore starts with identifying the cause.
When you should not wait for a routine appointment
If your bladder is full and painful and you cannot pass any urine, you need urgent care. Also seek urgent medical attention if urination involves blood clots and urinary retention, or if a high fever with chills is accompanied by a marked deterioration in your condition.
Which tests are actually needed
During the consultation, we will discuss how long symptoms have been present, previous operations, other medical conditions and all medicines you take. The IPSS questionnaire helps assess symptoms and later compare treatment outcomes. It does not establish a diagnosis.
Examination and urinalysis are supplemented by an assessment of residual urine and, where needed, uroflowmetry: measurement of urine flow rate. Ultrasound helps determine prostate size and other findings that influence treatment choices. A three-day bladder diary is useful for nocturia and frequent urges to urinate.
PSA testing is discussed separately, taking account of age, risks and how the result would change the next steps. MRI, cystoscopy and advanced urodynamic studies are needed for specific indications. Every man with a weak urine stream does not need this entire set of tests.
Monitoring, medication or surgery
If symptoms are mild and there are no complications, monitoring and changes in habits may be sufficient. We discuss fluid intake throughout the day, evening coffee and alcohol, and the effects of medications. When necessary, we select medication: different drugs serve different purposes and have different adverse effects.
Surgery is considered for severe symptoms, insufficient response to treatment or complications: recurrent urinary retention, infections associated with impaired drainage, bladder stones or effects of obstruction on the kidneys. The decision is based on the overall findings, not just prostate volume on ultrasound.
The procedure depends on anatomy, prostate size, health status and available technology. Before surgery, recovery, risks to urinary continence and possible changes in ejaculation should be discussed. These issues also matter when choosing medication.
What to bring to your consultation in Krasnoyarsk
Bring previous ultrasound reports, dated PSA results, discharge summaries and a medication list with doses. Bring any tests already completed; there is no need to repeat every investigation in advance. It helps to note how often you get up at night and which symptom troubles you most.

Frequently asked questions
Is a prostate adenoma cancer?
No, it is a benign condition. However, a person can have different prostate diseases at the same time, so the need for cancer assessment is considered separately.
Does a weak urine stream mean surgery is inevitable?
No. The cause must first be established and bladder emptying assessed. Some patients benefit from monitoring or medication; if complications develop, discussion of surgery should not be delayed.
Can I start with an online consultation?
Existing results and the next steps can be discussed online. The examination and tests needed for a final decision are performed in person. Urinary retention requires urgent care.
