A simple cyst and a complex lesion are different situations
A simple cyst is a fluid-filled cavity with a thin wall. Such findings are common, often discovered incidentally and do not cause problems. One or several simple cysts are not the same as polycystic kidney disease: that is a different condition, assessed in the context of the overall picture and family history.
If the report mentions septa, a thickened or irregular wall, or a nodular component, the urologist clarifies the features of the lesion. This is not yet a diagnosis of malignancy. Sometimes reviewing the existing images is enough to answer the question; sometimes a contrast-enhanced study is required.
Why the report mentions Bosniak
The Bosniak classification helps assess risk from the structure of a cystic lesion on CT or MRI, including contrast enhancement. The category is not determined by diameter alone. The report should ideally specify the classification version used; the doctor compares the category with the images themselves.
Reliably established Bosniak I and II lesions are benign changes that usually require no specific follow-up. Category IIF requires monitoring according to an agreed plan. Categories III and IV require discussion of cancer risk and the next steps; a single line in a report is not a reason to automatically promise kidney removal. Different approaches may be appropriate for individual patients, considering lesion size, health and the expected benefit of treatment.
When a cyst needs treatment
For a simple cyst, intervention is considered if it causes symptoms or complications, such as impaired urinary drainage. First, the doctor assesses whether the pain is actually due to the cyst: a stone, a spinal condition or another cause of pain may be present at the same time.
A symptomatic simple cyst can be treated with percutaneous or surgical methods. The choice depends on its location, structure and the specific treatment goal. A suspicious cystic lesion requires a different plan, based on the principles of renal tumour management. There is no universal rule that every cyst above a certain diameter must be operated on.
Which symptoms require urgent medical attention
A cyst mentioned in an old report does not automatically explain every new symptom. Sudden severe pain in the side or lower back, especially after an injury, requires in-person assessment. Fever with chills and pain may indicate infection. Visible blood in the urine also needs investigation.
Call 103 or 112 for fainting, sudden severe weakness, inability to urinate because of clots, or a severe general condition. If you feel well and the finding was incidental with no warning signs, you can start with a planned consultation.
How to prepare for a consultation
Bring existing ultrasound reports and all completed CT or MRI studies, preferably with images on a disc or access to the files. Older studies help with comparison: increase in size and changes in internal structure are assessed separately. If creatinine or urine tests have already been done, or you have received kidney treatment, bring the results and discharge summaries.
You do not need to arrange a contrast-enhanced CT in advance simply because the report says cyst. First, the doctor will decide whether clarification is needed and which method is safest. After the consultation, the next step should be clear: complete the assessment, monitor at an agreed interval, or discuss treatment.

Frequently asked questions
Could a cyst be cancer?
A simple cyst is benign. Complex cystic lesions are assessed separately: their risk depends on structure and contrast enhancement. The first step is therefore to clarify exactly what has been found.
Does every cyst require a CT scan each year?
No. Confirmed Bosniak I and II lesions usually do not require dedicated follow-up. For other categories, the method and timing of follow-up are determined individually.
